Abstract: Introduction: Different indices have been proposed to assess the esthetic outcome of single implant-supported crowns. The Pink Esthetic Score/White Esthetic Score (PES/WES) rates different soft tissue and restorative parameters. Vilhjálmsson proposed a modification of the Implant Crown Aesthetic Index (ICAI). The Complex Esthetic Index (CEI) evaluates five different parameters for three ... read morecomponents: soft tissue, radiographic and restorative. The aim of this study was to correlate PES/WES, modified ICAI and CEI to the patient's assessment of their esthetic outcome. Materials&Methods: Twenty five patients completed a questionnaire to assess their satisfaction with the implant-supported crown appearance. Two photographs and a radiograph were performed. An independent examiner completed all indices for the 25 patients. Five faculty members from four different specialties (orthodontics, periodontics, prosthodontics and oral surgery) evaluated four different cases twice in a two-week period. Results: The Area Under the Curve (AUC) was 0.53, 0.7 and 0.56 for PES/WES, modified ICAI, and CEI, respectively. The intra-observer agreement was strong for all indices evaluated (p<0.001). There were no differences between specialists. Both soft tissue parameters and restorative parameters influenced the patient's perception. Conclusion: The modified ICAI demonstrated a higher specificity. All indices demonstrated a good intra-observer agreement. No differences were observed between specialists. Further research is needed to assess if any of the indices demonstrates a greater correlation with patient's perception to warrant a standardized clinical use
